Created and implemented processes for transitions of care for Beth Israel Medical Center to decrease readmissions in high risk patients
Monitored compliance by PA’s, NP’s, housestaff, and hospitalists with grant initiatives and provided feedback to individual providers who were not compliant with grant initiatives.
Selected and trained RN care managers in transitions of care for high risk patients
Worked with IT departments from various outpatient internal medicine clinics to ensure their EMR’s are available to us on in the hospital.
Worked with the Education committee and Nursing administration to implement new action plans for floor nurses and case managers to use when doing disease specific teaching
Collected and reviewed readmission data for grant report requirements
